You will have to file a police report about the missing Ghana card. This is done to get the document to send to the NIA.

They will give you a form, which you will need to pay 20 cedis for the card after being given the form. The police report will help to validate the truth of your claims.

Go through the commission of oath after you have filled up the forms that will be given to you by the NIA. The commission of oath is right beside the office of the NIA personnel.
