Dwamena faced a setback in 2018 when a medical examination for Brighton revealed his heart problems, causing a failed transfer. Despite this, he had a remarkable season with FC Zurich, coming close to a £13.8 million Premier League transfer that ultimately fell through.

Opting for La Liga, he signed a four-year contract with Levante, but after a challenging first season, he was loaned to Real Zaragoza in July 2019. A subsequent cardiac diagnosis led to his release from Zaragoza, but Dwamena, determined to continue playing, chose to undergo an implanted cardioverter-defibrillator in January 2020.

Returning to the field with Danish club Vejle Boldklub in August 2020, Dwamena expressed optimism about his situation, citing his faith in God’s care for his heart. Despite media reports, he remained resolute, emphasizing that God, not humans, protected his emotional well-being. His unwavering belief in returning to the field guided his decisions, even in the face of differing medical opinions.

However, a second heart condition diagnosis two months later led to the termination of his contract. After over a year away from the game, Dwamena joined Austrian club BW Linz in June 2021 but experienced another heart incident in November of the same year during an OFB Cup match. Recovering from an “electric shock” delivered by his defibrillator, he expressed determination to undergo further checks and potentially return to the field.

Despite rumors of retirement, Dwamena joined Albanian club Egnatia in December 2022, concluding his professional career on a positive note. He finished the 2023–24 season with an impressive record of 12 goals in 12 games and one assist, leading his team in goals scored.
