You will have to file a police report about the missing Ghana card. This is done to get the document to send to the NIA.
After you have been able to do that, you will have to send the documents given to you by the police to the NIA. One of their office can be found in Shiashi around Legon in Accra. Submit the forms to them.
They will give you a form, which you will need to pay 20 cedis for the card after being given the form. The police report will help to validate the truth of your claims.
